Cultural Competence in Mental Health Treatment Essay
Trimbleââ¬â¢s chapter on ââ¬Å"Cultural Sensitivity and Cultural Competenceâ⬠brings a number of questions to mind, the most important of which seems to be the following: ââ¬ËAre psychologists expected to know about all cultures or ethnicities in the United States today? ââ¬â¢ Multicultural course content is expected to enhance studentsââ¬â¢ understanding of different cultures before they enter the workplace. All the same, it is virtually impossible for a psychologist to learn about all cultures that he or she would be interacting with in the course of his or her career. I would love to believe that the stages of cultural competence development end with ââ¬Å"cultural blindness (Trimble). â⬠Nevertheless, I realize the importance of understanding all possible cultures that I would be interacting with during the course of my career as a psychologist. I must reach the stage of ââ¬Å"cultural proficiency (Trimble). â⬠As a matter of fact, I had a vague understanding of this goal even before I had read Trimbleââ¬â¢s chapter on cultural competency. For this reason I had begun to read and watch movies about foreign cultures several years back. Needless to say, it amazes me that people from foreign cultures hold beliefs that are sometimes very different from my own. Moreover, it strengthens my belief that psychologists must understand the different cultures that they interact with. In my opinion, empathy ââ¬â on the part of the psychologist ââ¬â is the foremost requirement in a therapeutic relationship. At the same time, however, it is clear to me that psychologists who understand different cultures would be especially empathetic to people who represent those cultures. As an example, Latino psychologists may be especially empathetic toward their Latino clients. Furthermore, the job description of a psychologist cannot admit discrimination, racism, or prejudices. After all, mental health is a requirement of all people. Yet another challenge posed by diversity is that of different moral codes that have been adopted by different cultures and/or races. While multicultural course content could go a long way in helping the psychologist meet the challenges of diversity, it is crucial for psychologists who are introduced to new cultures for the first time to interview their clients on the basis of their cultural beliefs in the first instance. I state this with confidence based on experience, as I have had the opportunity to interview an individual whose belief system I could never have comprehended if I had not asked her questions relevant to her culture. It was important to ask questions relevant to her culture because it was possible for me to consider that her thought patterns needed serious reordering if I was not aware that her thought patterns actually stemmed from different cultural beliefs altogether. Hence, it was vital to frame the interview questions such that the interviewee would be allowed to explain her cultural beliefs while describing her problem. A Case Study The following is an excerpt from an interview report to shed more light on the importance of cultural competence in the psychologistââ¬â¢s career. Mrs. A (2007) is a 69 year old Pakistani lady settled with her sonââ¬â¢s family in the United States. She has been a green card holder for the past six years. During an interview, she reported having had ââ¬Å"no problem whatsoeverâ⬠with the American health care system. After all, both her son and daughter-in-law are doctors. Mrs. A suffers severe depression from time to time. In her opinion, it is ââ¬Å"genetic. â⬠From the time she entered the United States, her family has helped her cope with the illness by ensuring that she had access to the doctors whenever required, in addition to medication. According to the senior immigrant, the attitude that her family has shown toward her illness in ââ¬Å"a foreign landâ⬠is, indeed, praiseworthy. ââ¬Å"This is how families from my part of the world are meant to behave,â⬠she adds. She further believes that it is her family alone that she can rely on in the foreign land. In the Indo-Pak culture, an individual must be protected and provided for by his or her family. Women must be cared for by their husbands and sons. ââ¬Å"Single women have no place in our culture,â⬠Mrs. A reported. Individualism is replaced by collectivism, as Mrs. A would like her family to look after her interests at all times. In exchange for the time and energy that she has invested in her family thus far, Mrs. A expects help from her family in times of need. She worries, however, that her family would not be able to meet her needs all of the time. As a matter of fact, the possibility that her family might one day find itself impotent in terms of helping her out is a cause of anxiety for the lady. The intense level of anxiety felt by Mrs. A often translates into severe depression. Even though Mrs. A has a large number of friends across the United States, she believes that it is shameful to depend upon friends in stead of family. This belief is also culture-ingrained, seeing that the Americans do not believe that it is embarrassing to ask friends for help. Mrs. A visits her friends quite often, and they visit her too. All the same, they cannot root out the anxiety that lurks in the back of her mind ââ¬â that, in fact, it is possible for her to be left without family to take care of her needs, whether they are financial or health care needs. The anxiety is intensified by Mrs. Aââ¬â¢s concern that the American value system might abruptly change the Muslim-ingrained values of her son and daughter-in-law. Also according to her, it is widely believed in her own part of the world that the American children have no respect for their elders. ââ¬Å"Parents are sent over to nursing homes; and we would never do thatâ⬠ââ¬â she stated. Conclusion As the excerpt from Mrs. Aââ¬â¢s interview report suggests, it is essential for psychologists to learn about their clientsââ¬â¢ thought patterns and belief systems through interviews with questions that are especially framed to shed light on the clientsââ¬â¢ thought patterns and belief systems in relation to their cultures. No doubt, ââ¬Å"cultural proficiencyâ⬠is essential in the psychologistââ¬â¢s career (Trimble). For this reason, the psychologist must conduct research to find out about all cultures that he or she may be interacting with. 1. Identify some of the problems likely to occur in a boundaryless organization like Newskool Grooves. What are the Advantages of boundaryless organizations? Some issues that have occurred at the Kiev offices, managers there said that their computer programmers find working with little structure is uncomfortable. They are used to the idea of a strong leadership structure and well-defined work processes. Trying to reach co-workers in other offices who practice boundaryless organization has become an issue for some managers. They said that when they call the office they say to wait until their meeting day which is an issue because they have an immediate issue and cannot always wait until they decide to get together. Developers working on hardware in different locations had a communication breakdown which required many hours of discussion to resolve which could have been avoided if they were all on the same track. The developers seldom met face to face and all made progress but all of them moved in different directions with the product development. An advantage of boundaryless organizations: Boundaryless organizations communicate mainly through email, phone and other virtual methods rather than more traditional face-to-face communication. The freedom to telecommute with international employees removes geographical barriers to productivity and allows for schedule flexibility. By organizing expert employees in groups and giving them decision-making authority, these companies can change quickly to meet needs and function efficiently in an ill-defined hierarchy. Employees no longer work in isolation but work as part of a team on broad, company-wide projects, quality management, just-in-time methods, lean production, and supply-chain management,â⬠reports US Legal. To be successful, you should feel comfortable in a chaotic free-form workplace and have an ease of working with people to orchestrate the incredible amount of networking required. Consider some of the cultural issues that will affect a company operating in such different parts of the world and whose employees may not be representative of the national cultures of each country. Are the conflicts you observe a function of the different types of work people have to perform? Looking at the conflicts in Newskool, like the difficulties in decision-making and employees fighting to do what they perceive is better compare to the others. It takes time for the company to come with one decision as all of the employees can participate in decision making. People that are so diverse will suggest different solutions, reflecting their different cultures. Maybe it will be good to have a proper discussion in decision making, and only few managers can have the power in making decision in the discussion. It is not a function of different types of work people have to perform. As people from different areas or departments can contribute their opinions and ideas in the same work that require one decision. Another conflict will be inefficiency. Yes, different people must perform different tasks in their organization, but Newskool is boundaryless. When Gerd, the founder and the CEO does not satisfy, he asks his employees to redo the work tasks. It will cause a waste in resources as Gerd perceive something differently from his diverse employees. Sometimes, it is good to have hierarchy as people can know who to refer, which department to function and so on. This conflict is a function of different types of work people have to perform. The employees that are sometimes so dominant and think their works are correct, refuse to listen to Gerd and do things according to their own way. 3. Based on what you know about motivation and personality, what types of people are likely to be satisfied in each area of the company? Use concepts from job characteristics theory and the emerging social relationship perspective on work to describe what might need to change to increase employee satisfaction in all areas. Types of people are likely to be satisfied are competent, anxious, empowered and open people. This based on self efficacy theory; this theory refers to an individualââ¬â¢s belief that he or she is capable of performing task. People who posses self efficiency believe that they are capable in behaving in a way that produce that outcome they want. In Newskool Groovers, their employees are participates in decision making and innovation, creative and competent people. Because technology plays a major role as a communication medium in the boundaryless organization, much work is done from a distance via e-mail, phone, and fax. Less work is done in traditional face-to-face settings. Virtual collaboration makes it easier to use the expertise of a broader range of individuals. With telecommuting, international employees are more easily made a part of all business processes. Employees often like the freedom that boundaryless work offers them, particularly with virtual teams and more flexible work plans, arrangements, and schedules. Due to that, employee in company satisfied with the area of the company. Characteristic employees in company are likely competent and strong team skills. Concept job characteristics theory a model that proposes that any job can be described in terms of five core dimension: skill variety, task identity, task significance, autonomy and feedback. Based on this theory, to ensure employee satisfaction in all area in Newskool Groovers are by creating and maintaining a common task and group climate to focus groups and teams on the tasks at hand and on overall strategies, focus is on how to move ideas, information, talent, and decisions where they are most necessary, increase individual skill to do a task, hence he or she is encouraged to do it, regardless of title or position, jobs are rewarded on the basis of doing the jobs, not for accomplishing the necessary work. This rigidity is removed in boundary less organizations, where the goal is to develop greater flexibility and responsiveness to change and to facilitate the free exchange of information and ideas. It is made up of self-managing and cross-functional teams that are organized around core business processes. The teams include employees from different functional areas as well as customers and suppliers. Advantages Boundary less organization is able to achieve greater integration and coordination. They are able to adapt to environmental changes. Disadvantages However, it can be difficult to overcome political and authority boundaries. It can be time consuming to manage the democratic process to coordinate the efforts of many stakeholders. Companies are moving toward network structures, a group of organizations which coordinate activities via contracts, not a hierarchy. The research established that 69% and 27% percent of women were reported experiencing psychological aggression and physical assault respectively. The results also showed that women who have been fooled into marriage without their knowledge experience minor physical assault, severe physical assault and minor psychological aggression. The findings, the researchers say can be used in creating programs that can be used to address the problem effectively in the city (Kyu & Kanai, 2005). The second journal article aimed to study how children are involved in adult domestic violence. The study thus aimed to examine a large number of factors in the family and the surrounding that may be able to explain the differences that are witnessed in children in terms of their responses to adult domestic violence. This study used demographic variables as the independent variables were recorded as ordinal. A total number of ten independent variables and four dependent variables were analyzed in the study (Edleson, Mbilinyi, Beeman, & Hagemeister, 2003).
